SMART syndrome: a late reversible complication of radiotherapy.

چکیده
A 43-year-ol d female had a cerebellar pilocytic astrocytoma operated 27 years ago. Reoperation and radiation therapy were employed in a recurrence 13 years ago. One week before admission, progressive drowsiness, disorientation in time and space, slurred speech and worsening of a residual right hemiparesis begun. Cerebrospinal uid (CSF) analysis and electroencephalography (EEG) were unremarkable. She improved without speci c therapy. Recently recognized stroke-like migraine attacks after radiation therapy (SMART) is a syndrome which consists of prolonged, unilateral neurological symptoms that are spontaneously reversible1-4. Magnetic resonance (MR) ndings are fundamental for diagnosis, characterized by transient, dramatic, but reversible cortical gadolinium enhancement of the a ected cerebral hemisphere1-3 (Figs 1 and 2). Appropriate diagnostic workup should exclude other diagnostic possibilities.

متن کاملRespiratory Complications after Early versus Late Stabilization of Femoral Shaft Fracture
Background: Respiratory problems are common after long bone fractures. The objective of the present investigation is to evaluate the effect of early fixation of femoral shaft fracture on the incidence of respiratory problems. Methods: The results of early and late stabilization of femoral shaft fractures were studied in 150 patients. The patients were divided into two groups according to the st...
متن کاملEffect of reversible inactivation of locus ceruleus on naloxone-induced withdrawal syndrome in paragigantocellular neurons in morphine-dependent rats
In this study, the effect of reversible inactivation of locus ceruleus (LC) on naloxone- induced withdrawal syndrome in paragigantocellular (PGi) neurons in morphine- dependent rats was investigated. For inactivation of LC, 1 µl of lidocaine (2%) was used and for induction of withdrawal syndrome, naloxone hydrochloride (2 mg/kg) was injected systemically. The results showed that in dependent gr...
متن کاملComparing of different normal tissue complication probability models for plan evaluation of radical radiotherapy for esophageal cancer
Introduction: Chemoradiation is a well-established protocol for management of localized esophageal cancer. However, some early and late toxicities are associated with this therapy. Plan evaluation in term of normal tissue complication probability (NTCP) may help clinicians to tailor this therapy more individually. Given the variety of present radiobiological models, the choice...
متن کاملUnexpected Maternal Convulsion: An Idiopathic Case of Posterior Reversible Encephalopathy Syndrome after Delivery
Posterior reversible encephalopathy syndrome (PRES) is associated with various clinical manifestations such as headache, blurred vision, confusion and tonic-clonic convulsion. Some of the predisposing factors for PRES include hypertensive encephalopathy, preeclampsia and eclampsia, lupus erythematosus, thrombotic thrombocytopenic purpura and long-term use of immunosuppressive drugs. This condit...
